Self-Pay & Out-of-Network

For all other insurance providers, we are an out-of-network practice. We are happy to provide you with a monthly 'Superbill' (an itemized receipt of services). You can submit this document directly to your insurance carrier for potential partial or full reimbursement depending on your out-of-network benefits.
